Marketing Material Request Form

This Marketing Request form is for printed materials and monthly e-blast submissions. Fill out this form for every event or program that you would like Marketing to provide for you.

For printed materials: Please give 2 weeks notice when submitting materials due to the high volume of requests. 

For monthly e-blast submissions: Please submit by the 16th of each month.

Dates/times/address SHOULD NOT be included as a part of this description. If your event is longer than a day, however, tell us how long it goes (i.e. Senior Showcase, Wills and Trusts Week, extended offers, etc.)
Does this program/event require registration?
If yes, how should they register?
Is this event/program taking place at your branch?
Address
Is this event/program open to the public/nonmembers?
i.e. Is it open for all age groups? 6th-8th graders? Seniors? 3-5 years old? 
If this event is free, type "FREE." If not free, include fee for both members AND non-members if applicable.
